Renters must submit this form to landlords to avoid eviction, city officials say

WISCONSIN — The CDC issued an order halting evictions due to the COVID-19 pandemic, but in order to qualify, you must fill out a declaration form.

If you're renting your home or apartment but can not make your payments due to the pandemic, you may have felt some relief when the CDC halted evictions.

However, you should know that to qualify for the CDC's COVID-19 moratorium on evictions, you have to fill out the following form.

According to a letter from Alderman Khalif J. Rainey, "evictions for failure to pay rent are legal if the tenants do not sign a declaration, despite the CDC moratorium."

Rainey also reminded renters that "there are activities and violations that will not protect renters from being evicted – such as things that threaten the health and safety of neighbors and the community, including nuisance and criminal activities."

If you are at risk of being evicted due to being unable to make your payments, fill out the form above and give it to your landlord.
