DHS releases names of nursing homes with active COVID-19 cases

WISCONSIN — The Wisconsin Department of Health Services (DHS) has released the names of the Wisconsin nursing homes with active COVID-19 cases.

As of May 13, there are 38 nursing homes within the state which have active COVID-19 cases. However, there have been 46 nursing homes affected, eight of them have had their patients fully recovered.

According to DHS, a nursing home is no longer considered active 28 days after the last positive case was confirmed.

Currently, there are active COVID-19 cases at the following facilities:

  • Autumn Lake Health Care at Beloit
  • Baldwin Care Center
  • Bay at Burlington Health and Rehabilitation Center
  • Brookfield Rehabilitation and Specialty Care
  • Cameo Care Center - Autumn Lake Health Care at Greenfield
  • Crossroads Care Center
  • Crossroads Care Center of Pewaukee
  • Edenbrook Lakeside
  • Evansville Manor
  • Geneva Lake Manor
  • Grancare Nursing Center
  • Green Knolls at Beloit
  • Heritage Square Health Center
  • Holton Manor
  • Jewish Home & Care Center
  • Kennsington Care and Rehab Center
  • Lindongrove Menomonee Falls
  • Lutheran Home for the Aging
  • Maple Ridge Health and Rehabilitation Center
  • Menomonee Falls Healthcare Center
